ViaTrajectoire : simplifier l'orientation des patients grâce au numérique

Having become the national reference tool for healthcare and social care orientation, ViaTrajectoire supports several hundred thousand professionals and users daily. Since its creation 20 years ago, Coexya has helped this major public platform to evolve in supporting smoother care pathways.

The Challenge
Our role was to simplify the orientation of vulnerable groups, whether hospitalized, elderly, or living with a disability, toward suitable healthcare (SMR, HAD) or social care (Ehpad, Ehpa) facilities, providing secure system for the sharing of confidential medical data between professionals.
The Solution
Coexya developed ViaTrajectoire, a portal dedicated to healthcare professionals and end-users, allowing them to search for suitable healthcare or social care facilities, this also allowed them to create and track the progress of orientation requests online.
The Impact
Initially designed for the Rhône-Alpes region, ViaTrajectoire has become a nationally recognised reference tool, and has been successfully integrated into the core services of France's digital health doctrine. A dedicated Coexya team continuously updates the tool as orientation evolves.
